In brief: Excision

A punch is a tool that removes a circular portion of tissue.
It can then be closed with a suture.

In brief: Trade a scar

The basic idea is this is trading a scar for another scar. It is a form of scar revision.
However, the scar you get from the punch procedure will usually be a better looking scar. It is not a guaranteed procedure though so this must be a part of the discussion you have with your dermatologist. In the right patients it can help the appearance of some scars.

In brief: Removal of scar

Punch grafting is a method for improving severe, deep acne scarring.
A circular tool called a punch is used to core out (remove) the deep scar and a punch graft of normal skin tissue (e.g. Obtained from behind one of the ears) is put in its place. Typically many scars are treated. After the punch grafts have healed the entire treated area can be further improved with dermabrasion or laser therapy.
