If i recall, louisiana, new mexico, hawaii, native american reservations, and armed forces.
The candidate must have a masters degree beyond their doctoral degree--the masters must be in psychopharmacology. They must have a lot of supervised prescription experience and must pass a tough national exam.

Great answer Dr. Michael. As someone currently going through this experience I just wanted to add that we cannot yet prescribe in Hawaii but can in Guam (just in case you ever wanted to visit there... :) Our training is taught by MDs and pharmacists.
