Quite often the results of noninvasive examination, mammography and additional tests performed in special circumstances, like ultrasound or MRI are enough to perform excisional biopsy as the definitive diagnostic and curative method.
Biopsy in these cases would get the entire cancer which can be studied under a microscope.

Excisional biopsy is generally not an appropriate first means for making a pathologic diagnosis. Almost all breast diagnoses can and should be made via a needle biopsy before proceeding to the operating room.

The triumvirate of screening is breast self exam, annual clinical breast exam, and annual screening mammography after age 40.
For high risk patients (i.e. Family history, prior findings of atypic, etc) more intensive screening with more frequent clinical surveillance including being followed by a breast specialist/surgeon and other imaging (i.e. Mri) may be indicated.
