In brief: Circulation?
Cold-induced blue discoloration (cyanosis) can be circulatory, allergic, due to cancer or autoimmune diseases.
However this is not a disease--it is a syndrome which can be caused by multiple disorders. This needs evaluation by a doctor.

This may not be abnormal, depending upon how cold your son is getting and how old he is. It is very common for some kids to get blue lips when swimming in cold water for lengthy periods of time.
