In brief: Prostate cancer

The main way to diagnose prostate cancer is with a biopsy of the prostate.
Most men have a biopsy due to an abnormal PSA; the higher your PSA the higher chance of prostate cancer. If you have a PSA of 50 in the absence of other issues you have a very high chance of prostate cancer. Most men have PSA in the .5-2.5 range; a pt with prostate cancer in the bones can have a PSA over 1000

In brief: Medical examination,

To make a diagnosis of any medical disease, we take history for any symptoms bothering the patient, do a physical examination of the areas of concern and then do further tests if indicated by history and physical examination.
So if you have concerns, go see your doctor who can help you. It is fairly easy to diagnose prostate cancer(commonly occurs in older(>50 years) males.
