In brief: Multiple strategies

First step is safe detox.
Then, often in early stage of recovery Methadone or suboxone maintenance or natlrexone can play a role. Delete phone numbers of all your using friends from your phone contacts. Change your phone number so that they can't call you. Get involved with na. Find something that will replace your habits and rituals surrounding the addiction. Talk to your doctor. Good luck.

In brief: Probably w/ help

Some patients of mine were able to just stop with mild enough difficulty that they did not need help, but most do.
Your doctor and therapist could each offer assistance and it would be specific to you. Your job will be to provide accurate feedback and maintain your level of commitment. Good luck, i admire your intentions.

In brief: N.A.

Addiction treatment is complicated.
It needs to be designed for the individual. I would suggest you either go to narcotics anonymous or contact an addictions counselor to learn you options.
