In brief: Anal intercourse

Receptive anal intercourse is by its nature the most risky sexual practice for HIV transmission.
While it is fairly common among both male-male and male-female couples, men who have sex with men and take different roles have been a population at extra risk.

Especially after reading Dr. Killian's response, it's worth remembering that it's the actual practices -- sexual and non-sexual -- and not how a man identifies himself or is identified by others, that present a risk for HIV transmission.

In brief: HIV Risks

Over 45 million people world wide are infected with hiv.
If we confine the answer to just the U.S. Then here are the current facts. Young men under 24 are the leading group getting HIV in the U.S. Men over 45 are next. The groups gaining rapidly are post divorce and menopausal women. Hiv risk is about behavior. Sharing needles and getting exposed to infected blood has no sexuality.

Sexually homosexual men who participate in anal intercourse are at higher risk than from other behaviors. But, in all studies of homosexual behavior only 45% of men who have sex with men participate in this behavior. The rectum is particularly permeable and thus at greater risk for infection from exposure to HIV than other areas of the body such as the mouth and the vagina.
