In brief: Depends

When a wilms tumor is small, it may not have much of an affect on normal kidney function.
As it grows, it will slowly impinge on normal kidney function, which can result in problems with the urinary tract, filtering of the blood, and possibly cause high blood pressure. Many times the entire affected kidney needs to be removed as part of treatment (this depends on many factors).

Agree with Dr. Diede, however it is very rare to be able to save affected kidney unless tumor is present in both kidneys, or patient only has one kidney.
