In brief: Not always thyroid

Normal for some people.
Concerning if just one side. I had one patient sent to me for possible thyroid disease because she had a protruding eye even though she had normal thyroid tests. I had her get an mri. Turns out she had a tumor behind her eye.

In brief: One of main causes

Some people are born with protruding eyes.
Some genetic diseases cause protruding eyes, but thyroid is one of the main causes of protruding eyes because of increased thickening of the muscles which causes the eye to protrude.
