A sprain sometimes hurts more than a broken bone, because soft tissues have more nerve endings than does bone.

Some would rate severe sprains as more painful, so it is difficult to quantitate. Every person has different responses to pain stimuli, so one person's severe pain is another person's bee sting. So, there are definitely cases where broken ankles are missed.
