In brief: Old inflammation.

A granuloma is a type of inflammation.
Granulomas can arise in response to some infections and to other inflammatory processes. This process is generally not associated with malignancy. When described as 'calcified', (like in an xray or ct scan), the reader of the study is generally indicated that the process is mineralized and is usually of long standing.
