In massachusetts no 2 are the same. Certain prestigious institutions & physicians are reimbursed up to 50% more than other equally good and sometimes better ones. All clothed in a cloak of secrecy. Who is to judge? Presently, some ins. Contracts have tiered co-pays, to steer patients to hospitals which are reimbursed less!

This is only one of many reasons that health care in the USA, despite excellent training of physicians, does not have the same excellent in outcome seen in many European countries in which health care is predominantly a right guaranteed by the government in a nationalized system, rather than allowing multiple insurance companies to dictate availability of care.
