In brief: Immune System

Cdiff is often the result of an altered immune system, brought on by other infection or even normal use of an antibiotic.
It is not the result of a dirty environment.

In brief: Unfortunately...

Hospitals strive to protect all patients from hospital acquired infections.
Unfortunately, c dificile has become a very common problem with spread of increasingly resistant forms. Most of these problems are linked to antibiotic use as they eradicate normal bacteria from the gut, but fail to eradicate the c dificile that's already in the gut. This overgrows and then starts to cause problems.
