Why is pre term labour likely to happen again with future pregnancies? Does the body not "know" how to carry to term? How do the shots prevent ptl?

In brief: We don't really know

Statistically, if you have had preterm labor your chances of having it again are increased, but this does not explain anything.
Similarly, some treatments appear to reduce the chances, statistically, but how they work is unclear. Effective and reliable treatment has yet to be developed, nor do we have an effective way to predict it.
