In brief: Be aggressive

Brushing and flossing of course, but adults lose more teeth as a result of periodontal disease than anything else.
Ask your doctor if there i are more things you can do to stop the disease such as rinses, antibiotics or even surgery.

In brief: Maintain oral health

Brushing and flossing twice a day will help keep your gum tissue and teeth healthy.
Follow your dentist's instructions on home care. Eat a sensible diet with not a lot of refined sugars in it. Cut down or eliminate soda from your diet. Don't drink sweetened drinks right before bedtime. See your dentist regularly, at least every six months. More if the dentist recommends it.

In brief: Waterpik

Agree with dr hanley.
Add waterpik to your daily hygiene. I would go on 4 month recall and see how you are doing with your cleaning. If after one year (3 cleanings) you are doing well, you might go back to 2x/year. Some patients require 4x/year or more. Good luck.
