In brief: Needs evaluation
There are a number of reasons why he has lower urinary tract voiding symptoms; ranging from a urinary tract infection, enlarged prostate, etc.
He should be evaluated by his transplant team or a urologist as soon as possible.

In brief: A problem
These are all very important symptoms in a kidney transplant patient. Many things could be going on; uti, pylelonephritis (infection of the kidney), rejection. He could also have an infection someplace else and be having problems with perfusing his allograft. You should call your transplant coordinator immediately and arrange treatment.
