In brief: Transient delirium

Delirium upon awakening from sleep suddenly is not uncommon in the presence of a prior brain injury such as a stroke.
This should only last a few minutes after waking up.

In brief: Yes or yes

Even non-stroke patients who are awakened at certain phases of the sleep cycle may experience "delirium" a brief confusional state.
This tends to occur during rem sleep which is the same sleep phase in which "bad dreams" may prematurely arouse people from sleep. It this happens often, you may need a polysomnogram (sleep study) which your doctor can order.

In brief: Dream?

Brief hallucinations just as you go to sleep or just as you awaken are quite common and generally considered a normal phenomenon. This would not be referred to as a delirium which is not a transient state and has many more symptoms.
