Ana screen positive, ANA titer 320, rash very often on face, nucleolar pattern. Any significance?

In brief: Yes

The pattern is not important but a facial rash and a positive ANA suggest the possibility of some form of lupus.

I urge a rheumatology consultation to have more specific lab tests done. Also, a skin biopsy would be very helpful.

In brief: Yes

This is a significantly elevated title of ANA the rash may be indicative of an autoimmune disease like lupus.
More information is needed as to other symptoms you have and other lab values there are. You need to see a rheumatologist.
