Straight back syndrome is loss of the normal concavity of the thoracolumbar spine with a narrowed anteroposterior chest dimension, this can compress the heart giving the appearance of a widened heart (pancaked heart).
You can have an ejection murmur. The pain may be unrelated to the straight back.
