In brief: Piercing

Any piercing runs the risk of infection, excessive bleeding and of course pain.
A trusted health inspected and approved facility is essential to ensure proper sterile disposable instruments.

In brief: Yes, yes & & YES!

Tongue barbells can chip or fracture front teeth.
Gums can be damaged by the rubbing of barbells requiring periodontal surgery. If the barbell becomes unscrewed it could be aspirated into the lungs. Infections can be quite severe & there can be significant blood loss.
