Pressure transmitted through the spermatic cord while coughing can often be perceived as a dull ache or spasmodic pain in the scrotum. An inguinal hernia can allow such pressures to travel from the abdominal cavity with cough or strain.
A varicocele, or dilation of the blood vessels that drain the testes, can also cause similar discomfort.
