In brief: Possible

I apply Emla (lidocaine and prilocaine) cream to the area i plan to excise for an hour prior to the procedure.
This cream absorbs into the skin causing anesthesia. Then the area is injected with lidocain so the lesion can be absorbed. Generally there is no or minimal pain.

In brief: Not neccessarily

We often use topical or injections of topical anesthetics to make the procedure as painless as possible.
