In brief: Anastrozole may help

Some men with low testosterone (t) levels and high estrogen (estradiol or e2) levels may benefit from aromatase inhibitor pills such as Anastrozole (off label use).
Aromatase converts t into estrogen, and overweight men have increased aromatase. Weight loss also helps. The Anastrozole should raise t and lower e2 levels, but rarely do we see big improvements in sperm, often only modest improvement.
