Appendicitis starts as middle of abdomen pain (peri-umbilical), often with nausea, change in bowel habit, may get vomiting, fever, pain centering in right lower quadrant.

A ct scan will differenntiate for sure but other than that a history and physical examination is usually first performed to determine the nature of onset and compatibility with various etiologies.
