In brief: Oral Thrush

I suggest you first get a second opinion about your diagnosis as your condition is resistant to traditional treatment.
There are many other diseases that can cause white patches on oral mucosa and/or tongue.

THe yeast that causes oral infection is present in any warm moist area, including the genitalia, especially after conventional antibiotics for bacterial infection. This yeast can be cultured regularly even in absence of infection, so other causes of the findings need explored. And swollen glands due to yeast are quite uncommon. A monthlong yeast infection begs consideration of other diseases.
