Based on history, examination and exclusion of other causes.
Looks like a duck, quacks like a duck, most likely ... If it gets worse or symptoms change you should follow up though. Viral infections can lead to bacterial infections and strep is the one we are most concerned with. Mononucleosis is the only virus we usually test for and the symptoms are similar to strep except with more lymph nodes.

Most sore throats are indeed viral so if your doctor does not see exudates (white spots), he will not think of strep throat or diphteria.

Sxs of strep throat can include: pain of throat; large, red tonsils w white exudates; small red dots on roof of mouth; swollen lymph nodes on neck; fever, headache, rash, difficulty swallowing, fatigue or rash.
