Why do we give 1/2 dose (150) inhaled tobramycin, twice a day? Could 36lb kids get a full dose once daily to same effect? (trach w colonized pseudom)

In brief: Antibiotic dosing

Your body breaks down antibiotics at a steady rate, and it takes a certain amount of antibiotic for it to work.
If you were to only dose the tobramycin once a day, it will be too low to work in ~12-16 hrs; by using it twice a day, you keep it in your system & it works much better.

In brief: Works Better

Giving it twice a day allows the drug to stay in the airway at a higher level and thus work better than once per day.
Once per day tends to wear off allowing development of greater resistance to the drug.
