In brief: Difficult but common

It is difficult to intervene unless your family member wants help.
If the person is elderly, seeking a second opinion from a geriatric specialist would be a good place to start. It is amazing how many times my elderly patients improve just from stopping medications. However, this should only be done under competent medical supervision.
