In brief: Nurse
Most physicals especially in the very young are done by the school nurse to pick up major issues, vision problems and hearing problems.
It is not a complete exam but is designed to protect other students and to a small extent the one getting the physical. Physicals are more elaborate for the purpose of clearing students for athletic endeavor; usually done by a physician.

School Nurses DO NOT do physicals and are not licensed to do physicals. They do area specific screenings, such as vision, hearing, immunizations, or scoliosis. There is a significant difference between a screening by a nurse and a physical.
