Compare nursing homes by rating-- medicare website is one of them: http://www.Medicare.Gov/quality-care-finder/#nursing-home-compare also, when a nursing home has been surveyed by a state oversight agency, it should have the results of survey readily available for you to look.

What I am about to suggest is a rare combination of things and don't expect to get all of them together but here goes 1)accepts medicare and medicaid, low resident to nursing ratio, higher rn/lpn ratio, and one that has at least one activity or group per day and a responsible and dedicated medical director.
