One being the mildest and 4 the most severe. These stages mostly are use to correlate with the ability of the partient to get pregnant. Stage 3 has high infertility risk and should improve with proper surgical and hormonal treatment.

The scale depends upon the amount, depth, , and presence of pelvic adhesions and location of the endometriosis. It also predicts future pregnancy rates. Stage i is considered minimal, stage ii is considered mild, stage iii is considered moderate, and stage IV is considered severe.

The enigma with endometriosis though is that stage doesn't seem to matter for pain or infertility much. It's presence or absence of the disease. Age of the female is the most important factor in determining chances of pregnancy. That said, the very severe stage 4 cases do seem to have less fertility.
