Do not cut: The CR pill is coated in a way that controls the time/speed the medication is released. Cutting it does not make it in to an IR formulation, it just makes the way the medicine will enter your system very unpredictable-not a good idea for a med with potential for serious side effects. No "best" way.: A common way would be to take 5 mg for about a week and then stop. That would certainly avoid withdrawal effects. Yes, wrong: Intermezzo (sublingual ambien) is formulated for people who fall asleep normally, but wake up in the middle of the night & can't get back to sleep. It should also be taken only if you have at least 4 hours of sleep time left before getting up. No: Ambien (zolpidem) 10 mg is the maximum dose. The problem with ALL the tranquilizer type sleep meds is you build a tolerance to them and they don't work after a while. This is why it's best not to take these every night. Taking them every 2-3 nights works better. Alternative meds that could help with sleep are Trazodone, Remeron, or Vistaril. Other medications: Using Ambien (zolpidem) non-stop for that long would be expected to not work after awhile, because tolerance has likely developed. Ask your doctor about other possible medications that might pertain to your own case. I'm not sure if your sleep disturbance has been fully evaluated, as to why you need a medication like that for such an extended period? Having said this, each drug has side effects, so if you experience anything like drowsiness, irritability, or decreased libido, you might want to see your doctor for a dose adjustment, especially for Ambien, (zolpidem) as 10 mg might be considered too high a dose now.
