Not much: Both are selective serotonin reuptake inhibitors, or ssris, a fancy way of saying they help your brain hold onto and use more of the serotonin it has. Serotonin is one of the chemicals in your body related to happy things, like food, sex, and sleep (which is why the side effects often affect these issues).
Celexa, prozac, Paxil (paroxetine) are also in class.

Lexapro (escitalopram) works faster, in days, but sometimes makes people more "hyper" or "jumpy." zoloft needs 2-3 weeks to have an effect. They all work via the same mechanism -- inhibiting re-uptake of serotonin, thus increasing serotonin concentrations. The differences between the meds have to do with subtle differences in half life, serotonin receptors targeted, and slight differences in side effect profiles. Zoloft to Lexapro (escitalopram): Many physicians will do a cross taper gradually lowering the dose of one antidepressant while raising the dose of another. If however it has been determined that the zoloft has been of absolutely no benefit than there is no need for a cross taper. The Lexapro (escitalopram) should block any possible ssri discontinuation syndrome. Discontinuation symptoms with zoloft have been reported to be 1 to 2% but in my clinical experience have actually been far less than that. You should have no problem doing this. Good choice: My experience: lexapro is on the leading edge. It is a more precisely made molecule, derived from celexa. Acts more quickly than the typical ssri wait for 3-4 weeks and find it works fully in 1-2 weeks per a given dose regimen. Side effect profiles same as other ssri's. Other ssri's good too. I like to stay away from paxil (paroxetine) as found it too short acting-> irritability/anxiety. Talk to ur doc. ...Read more

A: A combination of cognitive behavioral therapy and anti depressants is an effective treatment strategy. There are other anti depressants with a different mechanism of action that could be tried- eg: tricyclic anti depressants (nortriptyline), mirtazapine, MAO inhibitors. The treatment could also be augmented with medications in the class of Abilify (aripiprazole), seroquel etc. If nothing else works, electroconvulsive therapy is a safe and effective treatment option. Probably not: In my experience, I have not seen Gabapentin to be useful in treating anxiety. Makes sure that your diagnosis is correct, and that you have been taking full, therapeutic doses of the medications you have been taking.
You may need a combination of medications, such as the addition of an antipsychotic.

Not benzo's.: Hello - long term use of benzodiazapines can lead to tolerance. Marijuana can cause marked psychiatric problems in some people including anxiety, psychosis & paranoia. Paxil can be used long -term but most of my patients have experienced more side effects with it then from Lexapro & zoloft. I see you are on Lexapro -it has an excellent side effect profile. Start Lexapro (escitalopram): While still on the zoloft. I usually have someone start on a small dose of the Lexapro (escitalopram) and increase it gradually while at the same time, taper down even more slowly on the zoloft. Each new antidepressant essentially has to start over again and needs time to work. Therefore you don't want to go off the zoloft too quickly, you may find out it had been doing much more for you than you had thought. ...Read more

Antidepressants best: Another antidepressant, possibly snri like Effexor (venlafaxine) may be more helpful at this time. Add ons such as gaba- calm and natural anxiety aids can help.
