Many: It goes without saying that you'll have to loop your neurologist or PCP in, but there are a variety with varying degrees of efficacy behind them that have been used as controller meds for migraines. Antidepressants: amitriptyline, venlafaxine. Antiepileptics: topiramate, divalproex. Other blood pressure meds: verapamil, lisinopril. Advice: I would be amazed if you have anything new other than your migraines evolving into a more resistant pattern. Analgesics of any kind are a BAD idea here as can result in chronic daily headaches due to rebound. Zomig (zolmitriptan) comes in a nasal spray and may work better, but other triptans can likewise work. Can we suggest a Concierge visit to help this further? ...Read more

Prevention: Try and identify then stay away from triggers. Avoid alcohol, avoid excessive caffein intake, if you smoke then please try to quit! Preventative mediations such as topiramate or valproic acid are effective. Acupuncture is actually as good or better at prevention than medications but can be expensive. Cambia only: If you've used 200mg of sumatriptan in a day you've used your limit, no more triptans (that includes zomig (zolmitriptan) and sumavel) today. Cambia is an NSAID, so that's ok.
