If you're taking it for a blood clot, the dose is a little higher at first, when the clotting propagation (getting larger) risk is highest, and then can be reduced after things stabilize a little bit. Normal liver/kidney: The normal half life is about 9 hours with normal organ function and 13 hours in the eldery. That is half life so a safe bet is 18 hours to 24 hour in someone.
