Question unclear: If you have an enlarged spleen you need to be evaluated for the cause, and to avoid trauma to the area of the spleen so that it minimizes the chance that it will rupture. The enlarged spleen alone is not a reason to go to the er, but if you think it may have ruptured, that is a medical emergency and yes, if not cared for can be fatal.
