Depends where ulcer: Brisk bleeding may result in hematochezia (passing blood per rectum), but more likely present as hematemesis (vomiting blood) or melena (black stool). Patients with bleeding ulcers above the end of stomach (pylorus) will likely vomit blood sooner than it will be seen as rectal effluent, whereas ulcers bleeding far down from the pylorus may first manifest their bleeding in stool. Pertussis: My best 'guess' would be pertussis or 'whooping cough' which is classically associated with vomiting after coughing. Coughs from this illness can be spasmodic and can last for weeks. Given your tests are reassuring, I think dealing with symptoms is the best treatment: Try regulating what you eat at when-- cough is worse at night. Try anti-cough medicines. Very unlikely: It sounds like you are on the right track. The PPIs have helped, but you still have some residual reflux symptoms. This is common and may be improved by diet or other lifestyle approaches (elevate head of bed, etc). However, with persistent symptoms you should have an upper endoscopy (EGD) to assess the esophagus and stomach on treatment, and this procedure would also screen for gastric cancer. ...Read more
