Most commonly blood in vomit may come from an ulcer, inflammation of the esophagus or stomach, or a tear in the esophagus usually associated w/vomiting. Mallory-Weiss tear?: A mallory-weiss esophageal tear may develop after vigorous retching. Brisk red bleeding however should not be ignored or dismissed. Old blood may look like coffee-grounds, and be dark-colored. Oozing, small volume bleeds may be diluted by other stomach contents and may look like streaks, clots, or pink-tinged material. 911 : 911 might be prudent. She could have esohageal varices, a peptic ulcer, gastritis, esophagitis - who knows. My concern is that from the way she sounds right now, she may have had a sigificant blood loss. Something like this happened to one of my patient's a little over a year ago. He told his kids not to call an ambulance, when they finally did call, he had died from vomiting blood by the time that rescue arrived. ...Read more

Management : You need to be seen immediately by a Gastroenterology it GI physician to manage your heartburn/esophagitis. You also need to see a Hematologist regarding your coagulopathy and ensure they coordinate care with a Rheumatologist as well.
