Well ... (go to doc): I think that several of the more serious possibilities - incarcerated hernia, orchitis & epididymitis, were ruled out by your lack of swelling. And tortion & cancer could be r/o by scrotal ultrasound, which I'm assuming you received. That leaves following up with your doc, or even a specialist Urologist, as your next best way to get answers. Get it checked: The pain may be nothing more then a strain, but it could also be something more serious. Years ago i personally had a similar pain that continued to get worse. I finally had it examined and found a problem. Testicular cancer. If i hadn't had it examined and treated i would not be here today. Rest/Ice/Motrin: Testalgia (pain the the testicles) is a common condition seen by urologist. Unless you had some direct trauma to the testicle, i would attribute the described discomfort to "strain"...Advise to use scrotal support, ice to the scrotum (you can leave your underwear on), and antiinflammatory meds like motrin.
