Sinus: Migraine is not brought on by positional changes and pressure. That is classic sinus headache. It still hurts and it's not easy to find a way to 'cure'. You are doing good things with irrigation and the antibiotic for times it flares up into an infection. A reason to see a Dr: To be correctly diagnosed, you need to see a doctor for an exam. There's no way to tell w/o one. It could be as simple as sinus or as complicated as a cardio-vascular issue - or anything between. Have an exam. Sometimes doctors try prescribing azithromycin or other antibiotics, in the hope they might help -- but knowing they may not.
