Stop the doxy: If you think it is the doxy, then stopping it for a week or 10 days makes sense (and if everything clears consider trying it a second time just to be sure). Doxycycline can cause esophagus and stomach irritation (the chest tightness and stomach pain) and worry about what is wrong, the fatigue.

PVC's: In a healthy 31 yr old male, pvc's are almost never the cause of any major issues. Mostly likely, you're having some anxiety when experiencing pvc's (which is very common)! nevertheless, get a check up to make sure there isn't anything else going on. If you're checkup is clean, I bet those symptoms will go away after getting the proper reassurance.

Surprisingly: It is usually associated with other symptoms such as fatigue, heart palpitations, undue sweating headaches etc. It also comes and goes. If this applies to you it could be as simple as diet, particularly the junk. Symptoms are often just "alarm bells" ringing in a complex system and provide warning that all is not well.

Possibly: Shoulder and neck pain don't really "cause" chest tightness, but a herniated disc in the neck can sometimes cause all these problems, including chest pain/tighteness that can seem like a heart attack.
