First : First of all, congratulations! you don't mention what your weight was before you lost weight, but the fact you have continued to lose weight is will statistically help you to have a longer and more active life. You also don't mention whether you are male or female, so i will tell you generally what i tell my patients. No matter what the charts say that you should weigh, i always ask patients what weight they desire to be for the rest of their life. I then do all that i can to help the patient through diet and exercise to achieve that weight. It's at this point that if they have areas of the body where there's excessive fat or loose skin, then we normally recommend a minimally invasive procedure. By definition, any surgery that does not violate the body cavity is a minimally invasive procedure. Using local tumescent anesthesia and new techniques of sedation, patients can have a very comfortable and safe operative and post-operative experience. Normally a patient of your body habitus who was at one time significantly overweight, and currently has achieved "close to desired body weight", will want to undergo either liposuction and/or surgical skin resection. Most people who want to achieve an excellent result require both. This might require either a sliding or a reconstruction of your bellybutton in order to remove all the skin that needs to be removed, and at the same time preserve a natural location of the bellybutton. There is a new procedure which is available which allows you to have full contouring of your entire midsection, including the front the sides and the back, and at the same time remove the excessive skin giving you a long-term excellent midsection area of your body. Because of new techniques that are employed the normal time off from work after such a procedure is less than 10 days. This compares very favorably to traditional tummy tuck procedures which sometimes required 4 to 6 weeks off of work. The new newer techniques also have less complications and less discomfort, although they include a more comprehensive treatment of the midsection area. After such a treatment if you were to gain weight again you would likely gain it in a more proportional manner. Without a physical exam there is no way to determine whether you need this new combination procedure or only liposuction, or only a skin resection. Agree: Some people like yourself are also predisposed to being 'skinny' and your excess calories will only accumulate at your waist, but as the doc mentioned you likely have elevated testosterone levels (acne, pimples) and thus prone to central weight gain. Could be thyroid: Could be a thyroid or other medical condition, so see a doctor to get this evaluated. Also depends on your net total daily calorie intake. If calorie intake is less than calories burned on a daily basis, over time you should lose weight.
