What do you advise if I'm going to get the sleeve done - bariatric surgery - I found out I have a hiatal hernia is that procedure done together or separate?
Together: The presence of hiatal hernia is not contraindication to having a sleeve done at the same time. If you have severe reflux in addition to the hiatal hernia, most surgeon would push you to have a RYGB. However, few surgeons believe that just repairing the hiatal hernia at the same time will decrease the reflux in combination with the weight loss. Gallstones or ulcer: Nausea after gastric bypass is not normal. Most common is small stones or sludge in gallbladder. Also consider ulcer (must avoid aspirins and tobacco). Differential Dx: Personally, I doubt the headache, nausea and even the sinus pain is related to an unresolved infection on #10, especially if the rct was done by an endodontist and two months ago. As any other patient: It mostly depends on your genetic predisposition and drugs used by anesthesiologist, then procedure itself. Theoretically eye procedures and gyn surgeries has higher incidence of nausea, but with modern drugs, it is not significant as in the past. ...Read more

Depends on function: The therapeutic approach depends on colon function. An elongated colon may work perfectly well, & therefore require no "treatment." a sluggish colon may respond to prokinetics (designed to speed up movement of material through colon). Yes and Possible: Most narcotics used for moderate to severe post op pain can cause nausea. You may lose your appetite due to a decreased gut motility, and your pain. So, i'd only use the narcotics when absolutely necessary. I'd try an anti inflammatory compound, if ok with your surgeon, such as Ibuprofen and naproxen. Nausea has many causes without knowing yours, the treatment will be inadequate.
Options include GI stimulants e.g. Reglan
Anti nauseants e.g compazine, (prochlorperazine) scopolamine patch, ginger capsules, dronabinol, tigan
Diet change eliminate spicy foods, caffeine, grease, gassy foods and eat small frequent meals

Swallowed air.: Most belching is due to swallowed air. People with acid reflux and heartburn swallow much more often to clear the acid in esophagus. With a sleeve, you might still have reflux.
