Talk to doc but: First you need to take the Pantoprazole 30-60 mins before your first meal of the day. There is also concern that chronic use of this type of drug increases the risk in getting pneumonia and possibly traveler's diarrhea.
If taken only when heartburn starts, then a Histamine 2 reverse agonist (Pepcid, Zantac) may be more appropriate. ...Read more

GERD: For the GERD heartburn, one can take OTC Omeprazole 20 mg /d or Zantac with a diet that avoids acid foods/drinks. Often H Pylori can be inherited or acquired. A simple blood test can be done to rule this in.

May not be a problem: Many people have hiatal hernias without ever having problems. Anything that increases your intraabdominal pressure a lot, such as doing strenuous activity or coughing a lot, or obesity, can cause the hernia to get bigger. Decreasing those activities will reduce your risk of symptoms. Once you have the hernia, you can't prevent it. It won't resolve on its own. ...Read more

Lot's of people...: ...have hiatal hernias, whether detected by x-ray or endoscopy. Most are sliding type & appear intermittently, requiring no intervention especially when small. Pls call your doctor & ask him/her your question to get a most accurate response re: hernia size as well its reducibility. GERD: Good for you for giving up diet sodas! It is possible to have worse reflux from iced tea, but it is usually from the caffeine in the tea. If you're drinking caffeine free tea, it usually does not make GERD worse. As for the DexIlant, (dexlansoprazole) is best to take it 20 to 30 minutes before a meal. I would not recommend taking it at bedtime. Good luck. ...Read more

Do you consume caffeinated products, smoke, lay down after meals, etc, which all aggravate GERD? Do you take meds for other health issues? It may.: If symptoms of GERD are not daily at this time, I would try to treat it by avoiding particular foods which bring it on, such as tomatoes, onions, orange juice, and chocolate. Avoid caffeine and alcohol, and stop smoking if you do. You may want to tilt the head of your bed. Use OTC antacids like Tums (calcium carbonate) whenever you have heartburn. Yes: It is possible that the omeprazole that you're taking is not adequately controlling your reflux, and the symptoms may still be present. If you have been taking the omeprazole for at least two weeks and are not improved, I would contact the doctor who prescribed it and see what the next step is. Well, not sure: Why worse but premenstrually many things worsen, perhaps even eating habits. Ranitidine is a medicine that is safe and much better than long term effects of reflux.
