If the decalcification is very shallow it can be polished out, but this means removal of surface enamel. Refraction: Possibly from fluorosis, although municipal water supplies in areas where ther's too much Fluoride usually reduce Fluoride content to safe effective levels. Most probably because the enamel rods in your enamel are somewhat convoluted rather than being perfectly straight. This causes light to be refracted differently and gives appearance of white spots.
