Lesion on tongue: It could be an inflamed taste bud. Anything that is inflamed will turn brown due to the smoking itself. I would have an ENT check it to make sure it is nothing. Do not worry, it does not seem that big by your description. If you need larger removal, the worst case scenario, it may not affect your speech and swallowing. More than one likely: There are more than one type or mouth sores usually related to hiv. Most have more to do with being immunosupressed- like candidiasis (ulcers caused by yeast infections), cold and herpetic ulcers as well as leukoplakia ("hairy tounge"). Doubt herpes. Yeast?: This description doesn't sound at all like herpes. I would be more suspicious of a yeast (Candida) infection, especially if his red spots are on the head of the penis and he is uncircumcised.
