Why do I hear voices in my head that sounds like people talking around me and about me?

So dificult: I can't really tell you matter of fact whether there are voices or not. I do take care of patients with problems and I tell them one comforting thing. If you are in my office asking me if what you are experiencing is abnormal, the chances are, you know what normal is. The most disturbed people I have seen argue that they do not have a problem, the people around them do.

Not necessarily: Sometimes blockages within the arteries of the neck can cause "swishing" sounds. The medical term for this is a "bruit". In the neck this is most often from the carotid artery. The sound is created by turbulence in the blood vessel created when the blood is flowing past a blockage / narrowing. A very simple and non-invasive test for narrowing in the carotid arteries is a carotid ultrasound.

Neck "noise": This is a common experience for many people. It is usually associated with degeneration (think about arthritis) in the neck. This often results with normal aging but is also often the result of sports related injuries or the result of heavy manual labor. The problem can be worse when osteoporosis/osteopenia is present.

Otitis externa?: Your symptoms suggest an infection in the ear canal. You do not mention ear drainage. See a doctor and the ear canal can be cleaned out and treated with a topical antibiotics.

Arthritis: This usually represents some arthritis in the neck. The sound comes from the vertebrae not moving smoothly as you turn your head. Or flex neck. It may not be present all the time and there does not have to be any pain involved. If you start to have pain and cracking see your doctor. It is important to set your car headrest so the neck cannot snap way back. Protect your neck from jerking movements.

An infection, maybe: Sometimes sinusitis and ear infections can produce these symptoms. These can be diagnosed and treated by a doctor or nurse practitioner. Your doctor can also help evaluate you for other things that might cause these symptoms.

Evaluation needed: Several causes come to mind. You may have a collection of abnormal blood vessels in the brain known as an avm. You may have a problem with your ear or carotid artery. Best to see your family doctor.

Otolaryngologist: This may be an inner ear or labyrinthine problem with tinnitus being the sound you complain of. If symptoms persist consult an otolaryngologist (ear, nose and throat specialist) or a neurologist for evaluation and advice.

Turbulent flow: Anything that is obstructing your flow and cause turbulence can explain this. Not common but can - Location of stenosis (coarctation of the aorta) - Aortic Stenosis - HOCM etc

Could be nothing: But could be a symptom of carotid artery stenosis. Not to panic. Tell your doctor about it and let them listen to your neck for a bruit. They might order ultrasound of your neck to look at the carotid arteries as well, a non-invasive, not scary test.
