How is LH hormone produced? Is it necessary to have a growing follicle to have an LH surge? How often does ovulation occur after a positive OPK?
LH: LH is made in the anterior pituitary, it takes a whole book chapter to explain how. It is necessary to have a threshold Estradiol level to have an LH surge, so practically speaking it is necessary to have a follicle. It is unknown for certain how often ovulation occurs after surge, but conservatively it is likely at least 95% of the time.
Ovulation prediction: Ovulation predictor kits (opk) predict when you will ovulate. When you detect the first positive that means you will ovulate within the next 24 hours. That means ovulation takes place next day after positive test. Once you see the first positive, you don't need to continue checking as the test will stay positive for 2 days then turn negative again.
It's possible: If you are using clomid, (clomiphene) you may get false positive's if done to early in the cycle. You should also do BBT charting to assure you are testing at the right time. When your BBT drops then suddenly rises, that it the time of greatest chance of ovulation and thus the time to repeat the OPK test.
Yes: Although it is not common, it is possible. For women with decrease ovarian reserve, sometime ovulation can occur even earlier. The reason that happen is due to elevated fsh level, which stimulate follicle growth and ovulation occurs earlier than the mid of the cycle. But for you, if that only happen occationally, should not cause concern.
OPK: The accuracy of opk varies - sadly, the opk are more reliable the more regular your cycles are normal, meaning the women who need them most will get the least use out of them. 5 days of "lh surge" is not physiologic, so it's either a bad test or you have a very unusual ovulation.
The following day: If you were checking yourself every day and you first detected the surge on CD21 then the following day (CD22) is when the egg is released (or ovulated).
I got opk positive at CD 13 and peak at 14th and I got my opk negative on my 15th evening. Did I ovulated?

Yes: Ovulation usually occurs 14 days before your next period starts. In a 26 day cycle that would be day 12 and in a 27 day cycle it would be day 13. This is absolutely normal. Hope this helps.

OPKs: These just measure urinary leutinizing hormone. Take the number of days from start of one period to start of next and divide in half and count from day one of your period and start checking your OPKs 2-3 days before that day. Once you have a positive, you are likely ovulating in 24 hours. Save the left over kits for the next cycle.
I tested positive for a peak OPK on nov. 11th but did not see any CM until nov. 14th. Does this mean I did not ovulate until the 14th?
Test wins: Ovulation kits are accurate. Monitoring cervical mucous is very inaccurate.

Not specific: The spike noted in the urine is the LH surge, which occurs about 24 hours before ovulation. The temp change is very non-specific and can be affected by a lot of things. You can probably look back to see when ovulation occurred by when your period starts, which should be 14 days after you ovulate.
OPK test & ferning positive yesterday. OPK test positive, but no ferning today. What does this mean? What is estimated time of ovulation?
Use the OPK: Ferning is a very outdated way to assess ovulation, it is really of historical interest only. When the OPK turns positive, intercourse for the next few days is recommended. Good wishes.

As expected: You stop doing ovulation kits after it turns positive because it will stay positive for awhile. If you are having regular periods and your ovulation tests show you are ovulating there is simply no value or utility in repeating them. Once you get a positive result then stop checking for that cycle. The hormone it detects lingers.

Yes to ovulation: Ovulation is a series of events and not just a single point in time. The initiating event is a sharp upward spike in LH levels (that's what the OPK measures). It's okay if the signal is negative the following day. The process has started and you have a chance to conceive.

Regularity of cycle: OPK detects LH surge which occurs in several circumstances like periovulation. Temperature rises when progesterone rise. Both occur around ovulation. The best test for ovulation is cycle regularity. Most women with regular cycles ovulate 14 days prior to their menstrual period. This is a retrospective diagnosis. If a women is regular 28 days ovulation occurs day 14, if 30 days regular then day 16.
Not accurate: Odk are a good way to predict ovulation. Once the kit is positive, ovulation or the release of the egg occurs 26-40 hours later. Although generally accurate, the odk can sometimes have a false negative- you ovulated but the kit does not show it. Sometimes using another kit works or there is always the possibility that you are not ovulating. 3% of women have regular cycles but do not ovulate.
Not always accurate: Odks are a convenient way to help time ovulation and the timing of intercourse, they are not always accurate. They can occasionally be positive but other more accurate signs of ovulation, like ultrasound, Progesterone levels and endometrial biopsy indicate that you did not ovulate. Mostly, however, when positive, odks indicate ovulation. May want to try a different odk or other tests for ovulation.
Trust the pain: I would err on trusting the pain. But, if you are trying to get pregnant the issue is not the specific day, but getting sperm into the vagina on the five days before ovulation. So, whether ovulation occurs on the 21st or 22nd, you should be getting exposed to sperm on the 16th through the 22nd.
