My recommendation would be to see an orthopedic doctor and see why and where the pain is coming from. Obviously after 12 years the original surgeries are healed. The callus usually forms within a couple of months, but can take over a year to remodel. If the bump came on suddenly and was associated with severe pain, then you may have a hardware failure. If the bone doesn't heal, the metal will fatigue, sort of like bending a paperclip until it breaks. Once the bone heals, it absorbs the forces around the metal and repairs itself.
