Tell your doctor: Tell your doctor you want to be weaned off. Most doctors would be happy to help you reduce the doses gradually until you are off or on a lower dose. If the pain you are suffering is so severe that when the dose is decreased your pain is not controlled, a long acting agent is always a good idea. Methadone is a great analgesic. Side effects?: Has your neurologist considered recommending nortriptyline which generally has less intense side effects than amitriptyline (if side effects are hampering you?). Get help: A very gradual taper will help minimize with drawal symptoms. 120 mg is a lot of oxycodone if you have been using that much everyday. You need to get some help through a drug rehab program. At least outpatient and possibly inpatient treatment through a good substance abuse program will give you the help you need. ...Read more

Slowly: I assume that you are taking 40mg every 12 hours for a total dose of 80mg per day. You should wean slowly, usually by 10% increments. I would decrease to 30 mg bid for 2 weeks, then 20mg bid for 2 weeks, then 10mg bid for 4 weeks. If at any point you have true withdrawl symptoms, you can increase to the higher level briefly. Clonidine at low doses can reduce withdrawl symptoms. ...Read more

Wait longer: Because the shingles vaccine is a attenuated virus, i would recommend staying off of your Enbrel (etanercept) the week that you get your vaccine; and, for six weeks thereafter. The six weeks allows your immune system to develop igg antibodies to the virus, the most effectivbe antibody. I recommend getting no more than 15 mg of Prednisone during the time you are off your enbrel (etanercept). ...Read more

Not science: You are on a low dose but you're correct in not stopping it abruptly. If the tablet can be cut in half, take 1/2 tablet nightly for a week, and then every other night for a week and then every third night for a week,etc until you're down to one for the last week as this medication has a long half life and changes are best made weekly.
